Planning for your care in advance contributes to the quality of care you receive during your illness. The session includes: What is Advance Care Planning and Why is it important? Who can be a health care agent and what are their responsibilities? An education on advance care goals and healthcare choices for Life-Sustaining Treatment. Zoom meetings are available.
