Don’t get caught unprepared by a medical emergency. Medical emergencies can hit anyone’s life at any time, anywhere, and at any age. Having a completed electronic advance care plan records your healthcare preferences and end of life wishes, which informs your doctors about your choice, reduces the burden to those you love and contributes to the quality of care patients receive during their illness. On-boarding Session - Create an Electronic Advance Care Plan Account, Name health care agent and identify advance care goals and healthcare choices for Life-Sustaining Treatment. Second Session - Discuss My Thoughts, My love Letter to My Family, My Circle Finalization Session - Discuss other conversation starters to have with your doctor and family. Record and sign your advance directive. *Additional fees may apply for in-person meetings further than 30 miles from Columbia, MD.
